Acknowledgement

I agree that I will comply with all pre‐employment screenings, obtain a flu vaccine, and refrain from using colognes, perfumes or scented body products. I also understand that if I do not adhere to the above commitments, I can be dismissed from my assignment with WellSpan Health.
